Thesis: Writing and Question

For me writing a thesis statement was one of the hardest parts of the writing process. I was unsure of how to come up with the idea for a thesis statement, much less how to create it or where it should be included in the essay. By following this guide for writing a Thesis Statement, my writing experiences have become more rewarding. First, the thesis statement is the main topic or idea of the essay. It should directly answer the question the writer asks himself or herself, and is a roadmap for the essay. The thesis statement provides the reader with a specific guide to the writer’s argument. A strong thesis statement should show conclusions about the topic, indicate a point about the discussion, help the reader to see the main point of the topic, and is specific to show exactly what the essay is about. Second, to gain the idea for what the thesis should say if a topic for the assignment is given, change the topic of the essay into a question. Then answer the question with one or two sentences. The answer to the question will be the thesis statement. If no topic is given for the essay, or if the information for the essay does not ask a specific question, then a question must be generated, based on the information given for the assignment and the issue chosen form the assignment information to be explored. One way to accomplish this, the writer must brainstorm the topic. Then narrow or revise the topic down to take a position about the topic by deciding what you really want to say. Review this statement and ask a question about the statement. Another way is to collect and organize evidence and look for possible relationships, similarities, or contrasts to come up with a specific word by clarifying relationships of the topic. Explain what is meant about the topic, and then revise this information to make an assertation. Next look at the statement and ask yourself a question about the statement. The answer to the question in either example will be the thesis statement for the essay. The writer should ask themselves the following; Does the thesis statement answer a question? Would the reader want to challenge or oppose the thesis statement? In addition, Does the essay support the thesis? The thesis statement should be placed as the last sentence in the introduction paragraph of the essay, the last sentence conclusion paragraph, and the last sentence of the abstract in APA style format. In conclusion, once I implemented this guide into my writing process, I found writing a thesis statement to be much easier. It seemed to become a guide for the body of the essay. By following the guide to Writing a Thesis Statement, my writing experiences have become more rewarding.

Tim Burton’s Movie Expertise

Tim Burtons use of deferent movie techniques made one of the best directors existing. H made many movies, all very popular by his fans. His work Is considered to be some of the best, bringing Gothic humor and dark, but innocent tones to the audience. His works with Gothic fantasy, a genre almost never used in the past, is common in his movies. Some of his most popular gothic fantasy movies are Edward Chardonnays and the Nightmare Before Christmas. Tim Burtons use with in techniques with camera shots mostly made him famous. There are many examples to this.He used a variety of long shots, close-ups, and medium shots. A movie where I find the most of these angles used was Edward Chardonnays. In a scene In Edwards dark castle home, Peg, was trying to sell make- up products. She went up the hill, with Ion-shots establishing the setting in the scene. She drove up the dark trail up the hill, passing dead trees and a quiet, but leer area. She went up to the castle door, having a close-up to see the Intensity and emotion of her fear as she went Inside. She knocked on the door, but no one responded.She entered in herself, a long shot showing her walk into the castle uninvited. Peg went up a winding stairs, a medium shot following behind her, wowing from up her waist. On the empty CD floor, a long shot showed her walk over to a bed made of straw, with photos hanging by It. A close up on her face revealed her emotions as she examined the pictures. For the audience, the scene was predicting something, building up an event. Behind her, a medium shot showed the front of Peg, with a shadow moving in the back of the room, making a snipping sound.Peg looked behind her, as the flexure walked out of the shadow. She seems horrified, a close up looking at her face. A medium shot showed the figure to be Edward Chardonnays. A close up shot showed Edwards emotions, fear. He stood stiffly in front of her, the camera tracking both of them in a medium shot. Peg walked up to him, asking wh ere his parent's are. He responded with a close-up, â€Å"He never woke up. † Tim Burtons use of lighting is very extensive along with expert camera angles. Lighting was heavily used to emphasize the Joy and happiness of shots In Charlie And The Chocolate Factory.This was the key to the most Important establishing shot, the candy meadow. As the children and Wily Wonk walked through a dark, bland hallway, the lighting was from the back of the scene, where they came from. They mound a small door, and Wily Wonk crouched down, unlocking it. Light shown through the door as he opened It, revealing a large candy meadow, with a flowing chocolate waterfall and river. The children†s and parent's faces ere surprised, a high- key lighting the shot of the meadow.The camera moved, browsing through the many plants that were made of candy. The colors and lighting were bright, showing happiness and the Joy of the scene. Wily Wonk grinned, extending his arms to emphasize the scene's wond er. He let the children and adults go, as they browsed through the bright area for the candy. Another example would be from Edward 1 OFF Scissoring, where Peg brought Edward none in near car, driving by a ass's looking neighborhood. High-key lighting emphasized the bright grass, showing peacefulness and innocence.Children played on the lawns, while Edward watched the area with a mix of fear, wonder, and Joy. They passed bright colored houses, ranging from green to bright blue. Tim Burtons use of sound is another fan favorite of the director. The orchestra music ranges from Joy to fear, to being surprised. Dietetic sound includes eerie creaks, and other techniques for creating a mood for the audience to the movie. In the Charlie and the Chocolate factory, music and dietetic sound was used to display emotion in the shot and convey the message given to the audience watching the movie.In the candy meadow, a loud orchestra playing wondrous, beautiful music played non- didactically to set mood for the shot. As the character's moved around, they could could listen and hear dietetic music, like the chocolate waterfall, or the wind rustling the edible grass and trees. In Edward Chardonnays, non-dietetic music was used in the scene where Peg finds Edward. It was slow, and curious. When they both were riving to pegs home, it change dint a fast, more Joyful sound for the audience.Tim Burtons expert use of lighting, camera angles, and sound made his movies what they are now. Starting in the sass's his quality stayed the same, if not increasing over time. Even to his new releases such as Charlie And The Chocolate Factory. His audience grows eve large, as he spreads his techniques to new forms of film. In his most recent work, he directed the music video â€Å"Bones† by the British alternative rock band The Killers. Along with this, he found a student's short animated movie, turning it into a full-length feature film, called â€Å"Nine. â€Å"

Bulgaria Economy Essay

Alongside the north coast of Black Sea, the Bulgarian state was first founded in 635 A. D with the Slav and Bulgar peoples. Between the year 893 and 927 the first Bulgarian Kingdom was established which is known as the â€Å"Golden Age† in the history of Bulgaria. In the early 20th century social and political disorders blemished Bulgaria and forced the country to participate in the First Balkan War in 1912 and then in the Second Balkan War in 1913. Bulgaria also took a part in the World Wars first by associated with the Central Powers and then with the Axis Powers. In the beginning of the Second World War, the country united with the Germany, but towards the end of the war it changed its position for fighting with the German troops that resulted in the killing of 30,000 Bulgarian soldiers. Communism came forward and materialized dominantly in the political environment of the country in 1944 when the Soviet soldiers entered into the territory of Bulgaria and the Axis power crushed in the Second World War. Gradually the country turned as the protectorate of the Soviet Union during the era of Cold War. With dethrone of Todor Zhivkov, thethen head of the Communist Party of Bulgaria, democratic change took place in the country. In 1991 the first egalitarian parliamentary elections occurred in Bulgaria. In 1992 the country experienced its first ever direct presidential elections. Bulgaria, which comes out through the agonizing struggle and trouble of communism, faced various social disorder and economic turmoil that ultimately ended with the huge economic and financial crisis in the period of the late 1996 and the early 1997. However, the economy of Bulgaria came back to the stabilizing path with a series of reforming initiatives taken by Ivan Kostov, the former prime minister of the country. (Background Note: Bulgaria, November 27, 2009). Overtime, Bulgaria has gradually emerged as a market-driven economy from an old command economy. In 1996, the country experienced major economic crises in the form of banking system disorder, depreciation of currency, reduction of production as well as foreign trade. Dwindling foreign exchange reserves and significant increase in rates of interest further aggravated both the burden of domestic debt and growth of the country. Within a period of one year gross domestic product dropped by almost 11% in 1996 while it experienced a 2% increase in 1995. In the late 1996 privatization model was accepted, but the privatization of the public sector industries declined. During this period the IMF disbursed almost $580 loan to Bulgaria. But the country failed to augment its structural reform process. This lack of progress of Bulgaria insisted the IMF to postpone the loan sanctioned to the country. For avoiding needless spending and huge inflationary pressure, the IMF proposed to form a currency board to renovate the confidence of the country. In this case Mr. Ivan Kostov (UDF), the former Prime Minister of Bulgaria, who got the crown in 1997, played the leading role to launch the ambitious reform programs. With such initiatives the currency board was set up on July 1, 1997 to bring back the stability and growth in the economy of Bulgaria. The outcome of the formation of the currency board was seen by the fall in inflation rate. While the rate of inflation was a three digit number in 1997, it was slashed to 1% in 1998. A decrease in the inflation rate, a decrease in interest rate and an increase in the foreign investment were followed by the formation of the currency board. The government of Bulgaria also assured to sell off some of the assets of the country. Though the country faced a 7. 4% drop in its GDP in 1997, it expected the GDP to bounce back in 1998 to an estimated level of 2%. The government of Bulgaria also took some other initiatives like achieving the success of land reform policy, the process of privatization and the reform of the banking system, etc. as its other prime objectives. The government of Bulgaria has taken the steps to provide a strong and sustainable growth in the GDP of the country by learning the lessons from the fall of GDP in the years 1996 and 1997. For reducing the level of unemployment and sustaining the growth of production of the economy, Bulgaria has taken all the required steps from time to time. The governments have taken the necessary measurements to reduce the level of both the individual and the corporate taxes, curb the corruption and draw the flow of foreign investment into the country. The government of Bulgaria has also taken the steps to restructure the foreign debt policy of the country, restore and stimulate the stock market and aggravate the process of privatization of the some of the major state monopolies. All such steps of the country have led the European Commission to declare that Bulgaria is â€Å"Functioning Market Economy† in October 2002 as its main economic policy. (Background Note: Bulgaria, November 27, 2009). All the successive governments have maintained such reform and ultimately Bulgaria has taken the membership of the European Union in 2007. A World Bank report suggests that Bulgaria has been able to draw the maximum levels of flow of foreign direct investment among all the nations of Eastern Europe as a percentage of GDP in 2006. The government of Bulgaria has brought down the rate of corporate tax to 10% in 2007 to magnetize the additional flow of foreign capital in the country. Such a corporate tax rate has been reported as the lowest rate in the whole Europe. The cost of domestic labor has also dropped through the legislation of the flat personal income tax level of 10% in January 1, 2008. To enhance the stability in local economic functioning, the Bulgarian parliament has taken the policy of fiscal decentralization for the municipalities so as to provide them the power of collection and supervision of some taxes. Such a step was taken by responding the increasing demand for independence of financial procedures of the local governments of the country. The achievement of the country towards the restructure of its fiscal policies and tax reforms, however, somewhat wiped out by the recent global financial crisis and economic turmoil that started from the middle of 2007. Such a worldwide global dwindling phase has forced the Bulgarian economy to be trapped by the vicious circle of recession at the end of 2008 after experiencing a prolonged 10 years steady growth. The effect has been seen in the mounting household debt and the increased rate of unemployment in the country. In this situation the government has responded by taking an â€Å"anti-crisis† strategy for strengthening fiscal recovery and promoting economic stability. To increase the power over the funds of the European Union, the government has also given its commitment. Such governmental steps are also expected to fight the crime and the corruption in the Bulgarian economy in an organized way. (Economy in Bulgaria, n. d. ). Let us look at some of the basic statistics which are very helpful to assess the present economic structure of the Bulgarian Economy and its overall development. It is a known fact in the history of mankind that the identification of the goals of economic development has not been properly done by human. (Todaro and Stephen, 2009, p 16) The Bulgarian economy has been designated as an economy with an upper and middle income nation by the World Bank. Though in the recent past the country has exhibited a steady trend in its growth, it is one of the least developed nations of Europe. The economy of Bulgaria heavily dependent on the industrial sector, but in recent past the service sector has also contributed to the growth of the country’s GDP. Petroleum, copper, gold, coal, beverages and tobacco, fruits, electronic tools and items, vehicle machineries, nuclear fuel, iron, construction items, sunflowers, wine, barley, etc. , are some of the major products of the country. From 1996 to 2008, over a period of 12 years, the country has able to maintain a growth of 6% and more. The huge inflow of foreign direct investment in the country has helped it to maintain such a significant rate of growth. However, the latest global recession has adversely affected the country on its export sector as well as on its capital inflow. This in turn has affected the growth of the country. The GDP of the country has fall to 5% in 2009. In terms of purchasing power parity, the GDP of Bulgaria has dropped to $90. 54 billion in 2009 from the level of $95. 1 billion of 2008. In terms of absorption of the work force of the country, the agriculture sector contributes 7. 5%, whereas industry contributes 27. 6% and service sector contributes 64. 9%. According to the estimation of 2009, the total work force of the country is 2. 63 million. The share of unemployed population in 2009 stands almost 9. 1% in the total working population. The corrupted system of public administration, feeble judiciary system and the increasing criminal activities has led almost 14. 1% of the population of Bulgaria to live their lives below the poverty line and under a greater hardship. (Bulgaria Economy, n. d. ). The estimated per capita GDP of the country has dropped to $12,600 in 2009 from the level of $13,100 in 2008. The estimated percentage of GDP of the country in agriculture, industry and services sector are 7. 5%, 27. 6% and 64. 9% successively. The estimated level of unemployment of Bulgaria has risen to 9. 1% in 2009 in comparison with 6. 3% in the previous year 2008. Total estimated gross investment in 2009 of the country is 28. 6% of GDP. The estimated amount of public debt has risen to 21. 4% of GDP in 2009 from the level of 14. 1% in 2008. The growth rate of industrial production in 2009 has been estimated as negative and stands at -14% of GDP. The estimated foreign exchange reserve and gold has also dropped to $16. 49 billion on December 31, 2009 from the level of $17. 93 billion on December 31, 2008. In response to the unfavorable effect of the global financial turmoil on the economy of Bulgaria, the government has taken many steps to strengthen the economy. Such governmental initiatives are supported by various statistics. The estimated public debt of Bulgaria has dropped to 21. 4% of GDP in 2009 as compared to the 14. 1% of the year ago. Estimated rate of inflation in terms of consumer prices has also decreased to 2. 7% in 2009 from that of 12. 3% in 2008. The prime lending rate of the central bank of Bulgaria has been increased to 10. 86% in December 31, 2008 from that of 10% in December 31, 2007. The stock of domestic credit has increased to $32. 04 billion in December 31 which was $17. 03 billion in December 31, 2007. (Bulgaria Economy 2010, February 8, 2010). According to the latest available data as published by Nation Master, the external debt of Bulgaria has increased to $3488 million which is a very high amount. In terms of Gini index, which is the measure of degree of inequality in the distribution of family income, the country has been ranked as 103rd out of 133 nations in case of its equal distribution of family income. For analyzing the socio-economic structure and development issues of Bulgaria we should study the basic education system, healthcare system, urbanization process and migration and the strategies of the country to develop the economy in the presence of corruption. The minority communities of Bulgaria are protected by many international instruments according to the 1991 new Bulgarian constitution. Such a constitutional structure has established the superiority of the international law over the national law of Bulgaria. Such international norms and regulations have made the basic principles of the legislation of the country through which the government of Bulgaria is able to control and adjust the common rights of its citizens who vary across the languages of their respective communities, religions and cultures. However, there are many problems in the country associated with the minorities. Such problems are not only related with the cultural or religious variations but also basically with the economic and social values of the citizens. (Karasimeonov, n. d. ). The government of Bulgaria is very concerned to respect its citizen’s human right. In past, there were, however, some problems in many areas. The citizens who were the member of minority population of the country were suspected by the law enactment officials. The prisoners were also mistreated and misbehaved. The degree of arbitrary arrest, police violence and exploitation were very common in the country. The prevalence of corruption and other problems associated with the system led the judicial structure of the nation to suffer greatly. The press was also restricted to freely expose the views. In Bulgaria there were also many barriers on some of the religious groups of the nation. They were strictly deterred to present their cultural and religious outlooks. Communal harassment was very common in the country. Social discrimination between the men and women was very high and the violence against the women was vastly prevalent in the society. The increasing problem of child labor in the country have also setback the economy of Bulgaria. Throughout the ruling of all the political organizations in Bulgaria, corruption and crime has remained the basic concerning factors of all the governments. To fight against the corruption and to increase the awareness of its citizens, the government of Bulgaria has formed the inter-ministerial anticorruption commission in 2002. To combat against corruption though the Bulgarian government has taken various methods, the European Union, however, has argued that the country is still facing many problems associated with the presence of corruption in the economy. The Union has recommended for the requirement of rehabilitated efforts to tackle such advanced and sophisticated corruption. The education system of Bulgaria is basically national in nature, but also affected significantly by foreign controls. In the 19th century the educational system of the country was highly motivated and influenced by the impact of Soviet Russia. The influence of Western Europe and the American educational culture are also significant on the overall educational environment of Bulgaria. However, to bring the country’s educational system in the line of Western European process of education the educational system of Bulgaria has been reformed. With such initiative to reform the educational system of the economy, Bulgaria has supported the agreement with the European Union. (Bulgaria- Educational System- overview, 2010). Development is the concept of freedom of human. The Human development index consists of the status of health, education and the income (Todaro and Stephen, 2009, p 19-21). The Human Development Index of Bulgaria is . 808 which has given it the rank of 55 among the 178 nations. (Economy 2010). In the early 20th century the proportion of residents who lived in the cities with their counterparts who lived in the villages were merely 20 percent. In 1945 the proportion has increased to almost 24 percent and at the end of 1990 it has been seen that more than 6 million people resided in the cities while less than 3 million population of Bulgaria resided in the villages. Such a change in demographic movement is well enough to argue that Bulgaria has experienced the pace of urbanization with the passage of time. Since almost 66% of Bulgarian migrants has relocated in the same states of the country, so there has not been seen any sharp decline in the regional population ratio. The demographers, however, are very concerned over the decline of the population of the villages. They are very concerned over the skewed distribution of population towards the cities. A model of the marketing communications process is presented in the study of Sandhusen (2000); it is explained that the specific model has been developed by Merton Electronics in order to support the promotion of the firm’s products; in the context of this model, the main parts of a successful marketing communications process should be the following ones: ‘a) the design of the advertising campaign, b) the proposition of the campaign to the media, c) the presentation of the advertising through the media, d) the decoding of the advertising campaign’s messages, e) the response of the public to the advertising campaign’ (Sandhusen 2000, p. 437). Creating Hyperlinks To insert the button I clicked on insert-interactive images-flash button when the options screen came up I entered information such as; style, background color, button text, font, link, target screen and then name of the button. I then clicked OK. Creating Hyperlinks To link between pages I needed to create hyperlinks, to do this I first wrote the text that I wished the hyperlink to display I then highlighted it and entered the link into the box that appeared I then pressed enter Entering text To enter the text on my site I simply clicked where I wanted it to go and typed it in, I also changed the properties of the text i.e. alignment, color, size, bold, format, etc by highlighting the text and using the toolbar. Accuracy To check the accuracy of my work, e.g. all of my links worked and that there were no spelling errors in the pages I got my friend to read through the page for spelling errors and dead links. Here is a spelling error that was found. and here is the correction Back-up saving To ensure that I dint lose any work if the computer crashed I regularly saved my work, I did this by clicking on the file menu and then "save all frames" or "save" button depending on which file page I was working on. I have provided screenshots below of the buttons I used. I also saved it onto a cd incase the computer was damaged and the work was lost. ‘Dear God†¦' the mayor screamed, as he crawled out from under his desk and pulled open the draw to reveal a steel dagger point, ‘†¦forgive me,' he cried as he charged at the beast's stomach. The only spectator, a smartly dressed Indian who sat like a statue in the shadows at the back of the hall. The hideous beast knocked the weapon from his attacker's hand and leapt onto the mayor's back. Though well built, the mayor's struggles were no match for the beast, and he soon crashed to the floor under the immense weight. ‘I had a wife and child,' He begged for mercy as the snarling beast rammed his face into the blood-smeared floor, snapping his jaw in two. ‘So did my father, sir.' The Indian reminded the dying man, still there at the back of the hall but with a voice that boomed. ‘But†¦' the mayor begged for help from the Indian one last time, though his plea was unheard mainly because of the fact the mayor had a mouthful of blood and broken teeth. The mayor stared at the severed head of a pretty young female; whose body still sat upright over in the line of chairs. Mirrored in her eyes he could see the destruction of all those who he had known. The pretty girl had been the first to die, and he would be the last. Something punctured the side of the mayor's neck; he felt like he was drowning as his lungs filled with blood. He was left to watch the girl's face sink into blackness as his light was snuffed out in the bloodiest of ways. The Indian sat emotionless, his white suit unmarked despite the destruction which had unfolded in front of him. He watched the beast play with the dead mayor like a rag doll, and listened to its screams of pleasure, he had watched the carnage without any remorse. These people had driven his ancestors off their land and murdered his family, this was the only way in which their souls would be put at rest. When the beast ripped out the still warm heart of the Mayors did he allow a smile of satisfaction, then a command for the organ to be crushed. From his pocket, the Indian pulled out a charm which was small, black, and made from some sort of stone it was also in the rough shape of the beast itself. ‘Fugue Santen†¦' He began to chant in his ancient tongue, the beast stood still. The Indian rose from his chair and made his way through the bodies towards the beast. ‘Lasa imenzes†¦' his voice filled the hall as he worked the ancient magic with his chants. He held the amulet in front of him, staring up into the eyes of the beast, which then turned to walk toward the Indian, towering head and shoulders above the Indian, unable to avoid the Indians hypnotic stare. The Indian had finished his spell. The town hall was silent once again but suddenly, a blinding flash of the most immense light came from the charm the indian had in his hand. The light blinded the beast with its beauty. The beast raised its arms in front of its face in defence, and then slowly the beast dropped onto its knees and began to walk on all fours back toward the wooden carving from which it had sprung to life. The Indian sighed and knew he had to work fast to collect the souls of his victims, which hovered in the attic above him. Until the last soul had been captured the Indians quest for vengeance would not be over. He left the mayors soul till last for the Mayor had been his main target. The Mayor had been the instrument of the death from which the order for his family's death had come. The charm warm in his palm, soon grew uncomfortably hot as he used its powers to collect the souls of his victims, he needed to avenge his family who had been murdered by the hand of the settlers less than half a century ago. So called men of God, who had slain his family and left him to starve in the mountains. He would have starved, had it not been for the witch who had found him and looked after him as her own, while also teaching him the ancient native Indian magic needed to avenge his family's death. Now the act of revenge was complete, the Indian had no purpose in life, his goal had been accomplished. The Indian knelt down on the steps leading up to the town hall, pulled out the sacrificial knife which his ancestors had once used and gave up his life in the hope he would once again be reunited with the souls of his ancestors. As the Indians blood dripped down the steps, his spirit soared as it was reunited with his brothers and sisters. Earlier†¦ The Mayor waited for quiet, noting the next number on the item list. The Mayor noticed that a crack had appeared on the carving it ran straight down the middle. Suddenly a beam shot out of the crack, splitting the carving straight in two. Women began to scream people ran for the exits, but the doors were locked. Meanwhile, at the back of the hall an Indian man in a suit finished his pipe, then quietly moved in to the shadows behind him. His whispered chants going unnoticed as he began to rub at an object inside his jacket pocket. The first victim of the beast was the girl nearest, a beautiful young girl. She was in shock unable to move as the beast swung its razor sharp claws and severed the girl's head in one deadly accurate blow. Course Project Rough Draft MGH HIPAA violation case Jennifer Brummage Medical Law and Ethics In the health care business, there are certain standards and laws that have been put in place to protect our patients and their personal health information. When a health care facility fails to protect their patient’s confidential information, the US Government may get involved and facilities may be forced to pay huge sums of money in fines, and risk damaging their reputation. The Health Insurance Portability and Accountability Act (HIPAA) was established in 1996. This Act was put into place in order to improve the efficiency and effectiveness of the health care system. The HIPAA law includes a Privacy rule and a Security Rule. Hospitals, Doctors, and employees in the medical field are expected to adopt the national standards and aim to keep patient information confidential. When a hospital or medical employee fails to meet the standards set, lawsuits can ensue and they can be fined large sums of money relating to the incident. The Privacy Rule establishes national standards to protect individual’s medical records and other personal health information and applies to health plans, health care clearinghouses, and those health care providers that conduct certain health care transactions electronically. The Privacy rule requires appropriate safeguards to protect personal health information. The rule also gives patients’ rights over their health information, including rights to examine and obtain a copy of their health records. The Security protects individual’s electronic personal health information that is created, received, used or maintained by a covered entity. The Security rule requires appropriate administrative, physical and technical safeguards to ensure the confidentiality, integrity, and security of electronic protected health information. The Office for Civil rights (OCR) is responsible for enforcing the HIPAA standards. When a complaint is filed, it is the job of the OCR to investigate. OCR may also conduct compliance reviews to determine if the health organization is in compliance with the HIPAA laws. When the OCR accepts a complaint from an individual, they will notify the person and the covered entity named in it. Then both parties will submit information about the incident. The OCR will review the information to determine whether or not a violation has occurred. When violations have occurred and have been proven, the US Government will impose a fine that they see appropriate. When Health organizations such a private medical practices, hospitals, and clinics fail to meet the standards described in the HIPAA act, investigations, bad press, and fines are surely to follow. There have been a number of cases in the past few years that have been investigated for HIPAA violations. One of the more recent and highly publicized cases was that of Massachusetts General Hospital (MGH). On March 6, 2009 is was reported that an employee of MGH had removed from the hospitals premises a folder of documents that included the private healthcare information (PHI) of approximately one hundred and ninety two patients. The employee had removed the folder from the hospital’s medical records room, so that she could bring her work home with her in order to complete some paperwork. The information that was included in these files were documents that had billing encounter forms that contained the names of the patients, their date of birth, social security numbers, addresses, phone numbers, medical record number, the patients diagnoses and proposed course of treatment, their provider and the providers address and phone numbers. The folder also contained documents that included the practices daily office schedule for three days and the medical record number for 192 patients. The employee was aware that she was not permitted to remove this confidential information from the hospital premises. In doing so, she violated the HIPAA law. On March 9, 2009, the employee who removed the documents from the hospital was commuting to work on a subway train. According to the complaint that was filed, the employee had removed the folder containing the documents from her bag and placed them in the seat beside her. The documents were not in an envelope and they were bound only by a rubber band. Upon exiting the train, the MGH employee left the documents on the subway train. The documents were never recovered. This incident was later reported to the Office of Civil rights (OCR) by a patient who was informed by the hospital that his medical records had been lost by an employee and left them on a subway train. The One hundred and ninety two patients involved had been patients of the hospitals Infectious Disease outpatient practice, which includes HIV/AIDS patients. The fact that the patients involved in this case were potentially AIDS patients, made the violation that much more serious. Investigators had to take in to account that these people had their medical records lost, and in those records were their phone numbers and addresses and possibly their place of employment. If these documents fell into the wrong hands, the potential for destroying the patients’ lives was very high. Had a person with malicious intent got ahold of their information, they could have harassed the patient and possibly spread their personal information around, which could have had devastating consequences. The Office of Civil Rights began their investigation of Massachusetts General following the March 2009 Complaint. Because of the potential violations that MGH faced, they agreed to pay the United State Government $1,000,000 to settle potential fines. MGH is one of the nation’s largest and oldest hospitals. The Hospital is highly regarded and respected, and many hospitals took notice when the investigation into possible HIPAA security law violations began. In addition to agreeing to pay the United States Government one million dollars, the hospital and the General Hospital Corporation agreed to sign a Resolution Agreement with the United States Department of Health and Human Services (HHS). The agreement required that the hospital develop and implement a comprehensive set of policies and procedures to safeguard the privacy of its patients. In signing this resolution agreement, it was the HHS hope that other hospitals and clinics throughout the nation would recognize that the OCR is very serious about investigation every claim that is filed with them. The OCR wants other hospitals to see that if a violation has occurred and a patient’s privacy has been violated, there will be consequences. The OCR wanted to make an example out of Massachusetts General Hospital. In addition to the fines and the signing of the resolution agreement, OCR and HHS asked MGH to enter into a Corrective Action Plan. The HHR wanted the hospital and its employees to not only be held responsible and made an example out of, they also wanted the hospital to raise the awareness of its employees. The Corrective Action Plan (CAP) was designed to develop and implement a comprehensive set of policies and procedures that ensure the patient’s private health information is protected when Removed from the hospitals premises. It was to ensure that the employees were trained and informed of the new policies and procedures so that future mistakes could be prevented. The hospital was also required to have the Director of Internal Audit Services of Partners HealthCare System Inc. to serve as an internal monitor who will conduct assessments of MGH’s compliance with the CAP and render semi-annual reports to the HHS for a 3 year period. It was the mistake of one person that caused so many changes in MGH’s system. It was a costly mistake, but ultimately is has helped the United States Government make hospitals aware that if the standards set are not followed then there will be consequences. The HIPAA laws that are set in place are meant to protect patients. Even the US Government and the employees of MGH are someone’s patients, and they would also want their privacy respected. Hospitals across the nation, private practices, doctors and healthcare facilities should take notice, they need to make sure their employees are trained and informed of the policies and procedures regarding patient’s privacy and security. Every hospital in the nation should raise their own standards so that they are above the ones set for them. Patients will take notice and be appreciative and more trusting when receiving care. In conclusion, this HIPAA violation could have possibly been prevented had MGH implemented the Action Plan in the beginning. The hospital should have had a program that required all employees to take prior to starting work with the hospital. Had the employee that left the documents on the subway been through a class on HIPAA laws and the correct way of handling PHI, maybe the incident never would have occurred. Hospitals should hold a class as part of the hiring process to thoroughly train their employees on this issue.
